Caring for the community since 1995, Oxford’s first and favorite walk-in clinic is open seven days a week.
Note: When you pick your appointment time online, that makes us aware of what time you WILL ARRIVE at the clinic. If you have your forms filled out beforehand, it also improves your wait times. Be sure to bring a picture ID/driver’s license along with your insurance card(s). Also, the online check-in is only available from 8a-4p M-F, Saturday 9a-3p and Sunday 12p-4p. We still take walk-ins throughout the day and our office hours have not changed. We look forward to improving your visits and wait times while providing this availability.
If you cannot pick your time, please pick the next available one. If that does not work you are still welcome to walk-in for service.
If you fill this out prior to arriving at the clinic, it should help improve your wait times.
If you are coming in for a DOT Physical, filling out these forms prior to your arrival should reduce your wait. Be sure to answer all the questions that apply to you and completely bubble in the circles. If you are unsure or questioning your response to any of the questions, skip over them and our staff will be happy to assist you when you arrive. YOU MUST PRESENT A VALID DRIVER’S LICENSE UPON YOUR ARRIVAL OR WE WILL NOT BE ABLE TO COMPLETE YOUR PHYSICAL.
This is for Worker’s compensation. Any information that can be provided by your employer prior to you arriving will aid us in improving your wait times.
